Place: Tech Innovative Museum - 201 S. Market Street, San Jose, CA.
Date: July 23, 2019 – We are not responsible for children or adults not there when bus leaves.
Time: 8:30 am – Meet at 1221 Pacific Avenue, San Leandro, CA.
3:30:pm – Pick Up Children – 1221 Pacific Avenue, San Leandro, CA.
Grandmothers Who Help, will provide one ticket for admission and lunch per child and chaperon. This Ticket will not cover I Max Theater, Museum Cafe, or gift shop located within the museum.
My Child has permission to travel to the Tech Innovative Museum, 201 S. Market Street, San, Jose CA, under the supervision of Grandmothers Who Help, Inc. July 23, 2019. I agree to send a $10.00 donation for my child, on or before July 23, 2019 to help with lunch and transportation. To have my child at 1221 Pacific Avenue by 8:30am and to pick them up by 3:30pm.

(entity), its elected and appointed officials, employees and volunteers and others working on behalf of Grandmothers Who Help, Inc., (entity) against any and all claims, demands, suits or loss, including all costs connected therewith, and for any damages which may be asserted, claimed or recovered against or from Grandmothers Who Help, (entity), its elected and/or appointed officials, employees, volunteers or others working on behalf of Grandmothers Who Help, Inc. (entity), by reason of personal injury, including bodily injury or death and/or property damage, including loss of use thereof, which arises out of or is in any way connected or associated with this contract.
Children needing a chaperone to administer medicine must stay with parent until chaperone arrives.
